Duties: Responsible for intensive case management of customers in the re-entry population to focus on job training, building education and stabilizing in the community through employment and retention. To assist with overcoming the barriers to long-term unemployment/underemployment and family stabilization using Family Development strengths based approach. Triage caseload to ensure adequate support to individuals in crisis and in high need. Complete comprehensive assessments of family strengths and needs using agency tools and best practices. Develop goals and milestones with customer participation according to the Brief Solution Focused treatment model and Family Development in conjunction with the Employment Services Team and key partners. Conduct home visits and interact with customers at off-site locations to observe and support task completion, graduation, advancement and retention of training, education and employment and stabilization in daily living. Provide basic life skills training both on-site and at off-site locations such as in home visits to support successful community reintegration.

Requirements: Master's Degree in Social Work, Human Services, or related field. Two (2) years' experience providing related services to families and individuals. Preference will be given to those who have proven direct experience with Re-entry population. Knowledge of community based services, public assistance and other community resources and demonstrated skills in accessing these services appropriately. Must have the ability to learn quickly the resources available in Schenectady County, and be able to appropriately refer in-house working with other program support staff to ensure full access to SCAP's services. Must have a valid New York State driver's license.
