35 hours per week and 42 weeks per year.

Duties: Conduct routine height, weight, hearing and sight screenings within 45 days of child entering the program, and be able to do this with expertise. Review and assess for irregularities and/or follow up all children's physicals, immunizations and dental records and work with the Health Coordinator to develop a plan of action. Follow through timely on all directed activities and keep accurate and retrievable records of these follow ups. Maintain all health, dental and related information and ensure accurate and retrievable filing of documents. This includes timely and accurate data entry in CAP60 as well as maintaining accurate files for necessary paperwork in hard copy. Assist Health Coordinator in organizing and implementing routine staff training as assigned in a timely and professional manner. Actively assists the Family Services component in the annual recruitment process. Actively works with Family Service Workers and Home Visitors to follow up with Health related information including acquiring documentation, providing appropriate health referrals and following up on the status/follow through of referrals.

Qualifications: Must be a Licensed Practical Nurse with license up to date. Two (2) years' experience with preschool children preferred. Must be First Aid and CPR certified as a Healthcare Provider or be able to obtain certifications within the introductory period. Must possess valid New York State driver's license and have a clean driving record. Sensitive to the needs of low-income and minority populations and must display common courtesies. Physical and background check required.
